Scheduling your first visit

In order to better serve you, we ask our patients to bring the completed form below, their current insurance cards and driver’s license or other picture identification. Please bring any discs with X-Rays, CT, or MRI with you. If you have any questions regarding this matter, please call before your appointment.

If you are a new patient to our clinic, you may save time by completing the New Patient Forms prior to arriving for your appointment. If you have any questions regarding your appointment or our office policy, please feel free to contact our office.

For patients with HMO’s or PPO insurances that require referrals, we must have an insurance referral from your primary care physician before you can be seen by our physicians. If you were injured at work, we require a letter of authorization from your workman’s compensation carrier which includes the carrier name, address, phone number. This should also include the name and phone number of a contact person/claims adjuster. This information can also be mailed or faxed to our office prior to your visit. We do not file automobile insurance or accept letter of protection from attorneys. If you do not have health insurance, please contact our office prior to your visit to discuss payment arrangements. Our office requires payment for services rendered before leaving our office. Please bring any copay or deductibles that your insurance requires with you to your visit. If you are uninsured, please contact our office to discuss our payment policies and arrangements BEFORE your visit. Monthly payment arrangements may be offered to insure that your account is not sent to an outside collection agency or attorney. As a courtesy to you, our office accepts cash, personal checks, money orders, Visa and Mastercard.

1. Request an invitation at your next appointment and we will send an email with a link to the portal.

Get started…It’s as easy as 1, 2, 3 Click the registration link in the email you receive from Florence Neurosurgery & Spine Center. Choose the login method and accept the agreement to authenticate your account. Enter the invitation code provided by your medical organization and click “Agree” to release the information. You will then receive two emails: one welcoming you to the patient portal and one confirming your connections with Florence Neurosurgery & Spine Center. You now have complete and secure access to the patient portal: the latest advancement in personal health Management.

2. If you have not received an invitation email, but wish to create a Universal Health Record, please use the following steps:

Click here Click the "Pre-Register' link button where is states "Would you like to join our practice" Fill in the appropriate information(Please provide the accurate zip code as this is the measure we use to search for the local medical organization that connects to us) Log in. If you do not already have one of the shown logins, please use the link provided to create one. Click "Connect" next to the name of the desired medical organization(s). They will move from "Search Results" over to "My Organizations". You may need to adjust the distance range on this screen. Once all your medical organizations are selected, click "Next" at the bottom right to continue. Thank you for choosing to be proactive in managing your care!
